Tubal adhesion is a pathological change caused by inflammation stimulating fallopian tube.
Tubal adhesions contain adhesion of fallopian tube lumen and peritubal adhesion.
Adhesion of fallopian tube occur because of infection genital mucosa, from vagina to uterus, then endosalpinx. Ascending infection will result in tubal ectopic and isthmic obstruction;
adhesion of fallopian tube lumen will lead to infertility. Main
symptoms of tubal adhesion are abdominal discomfort, irregular menses, oligomenorrhea, dysmenorrhea and infertility.
Peritubal infections are caused by infection of lymphatic syatem. Bacteria can spread through pelvic connetive tissue and other parts of internal reproductive organs from vulva, vagina, cervix and lymphatic vessels of injured corpus. They are puerperal infection, postabortal infection and intrauterine device infection.
Patient’s tubal serosal layer adhere to peripheral tissues after hyperemia and edema, and it result in infections of tubal serosal and myometrium. Tubal patency can be displayed while doing angiography. Concurrent infection of fallopian tube lumen is also possible. In addition,
endometriosis and operation may reasult in tubal adhesion.
Peritubal adhesions are mutual adhesions of fallopian tube, ovary, pelvis which will lead to fallopian tube elevation, fallopian tube bend and affect tubal peristalsis, and fimbrial end adhesions. They are factors to infertility.
Even if inflammation has been controlled by antibiotics, tubal adhesion still exists, because tubal obstruction and peritubal adhesion are mostly pathological changes formed as a result of inflammation. Pelvic fallopian tube enlarged and uterine activity weakened or pelvic masses or straps exist while doing pelvic examination. Conditions of peritubal adhesions can be diagnosed by laparoscopy.
