Acute orchitis is always caused by heamatogenous infection or lymph-through infection, it could occur with many other acute communicable diseases. If you are suffering from an epidemic parotitis, the virus could be discharged with urine and causes acute orchitis.

Clinical manifestation of Acute Orchitis

Acute non-specific orchitisoften occur when you have a urethritis, cystitis or prostatitis. The infection would spread through the spermaduct to the epididymis and causes a epididymitis or orchitis. Common pathogenic bacteria is E.coli, proteus, staphylococcus, pseudomonas pyocyanea and so on. The bacteria can get to the testis with blood circulation and causes orchitis. When the disease occurs, the testis would be swelling, pain could be felt, scrotum is red, no urinary symptoms. Patients' body temperature could be as high as 104 F。

Pathophysiology of Acute Orchitis

Hippocrates first reported the syndrome in the 5th century BC. While the more common epididymo-orchitis is bacterial in origin, isolated orchitis usually has a viral etiology.

Diagnosis of Acute Orchitis

1 Signs: patients have the symptoms of epidemic parotitis or other acute communicable diseases. Testis is swelling in one or both sides and have a pain if pressing. The bounds with epididymis is unclear. Sometimes acute hydrocele testis occur with this disease.
2 Tests: Number of white cells is increased, urine routine is normal or with CRP, or blood in urine. Epidemic parotitis would do some harm to kidney function, some specific virus can be found in the urine culture.
3 According to clinical symptoms, signs and tests, this disease can be diagnosed. But it also need to be distinguished with acute epididymitis in clinical cases. In low stage of acute orchitis, inflammatory epididymis can be found, however, in advanced stage, the bounds between testis and epididymis is unclear. There would be no urinary discharge or pyuria.

If orchitis is not cured for a long time, the fibrosis and the damage in testicular follicle can cause a testis atrophy and infertility.
